  • Overview

    • To provide a time limited ‘bridge of services’ to stabilize consumers after discharge from the hospital and assist with the transition back into the community. 
    • To connect identified high risk consumers with adequate supports and services.
    • To intervene with consumers and offer extra support and assistance to avoid decompensation and prevent a downward emotional spiral leading to hospitalization.
    • To provide bridge services and outreach to new consumers after intake until 1st session with therapist/prescriber.

    Responsibilities

    To carry a partial caseload of Intensive consumers as needed, including the following:

    • Provide clinical/case management services including service planning, monitoring, follow-up, linkage and crisis management for consumers.
    • Provide services to meet need:  office based, community based, or in the consumer’s home.
    • Make appropriate referrals to and act as liaison with community agencies, service providers, and natural support systems
    • Collaboration with consumers, family members, and significant others in identifying strengths/needs in developing personal treatment goals
    • Knowledge of Stages of Change model, Motivational Interviewing techniques, and Cognitive Behavioral Therapy skills.  Knowledge of Recovery-Based Treatment and Trauma Informed Practices.
    • Work with consumer and family/support system for appropriate service delivery
    • Provide assessment of daily living skills (ADL’s), coordination, monitoring, planning, referral and follow-up with comprehensive community services and supports
    • Provide individual, group, family case management services and therapy as indicated and identified in the service plan
    • Provide assistance in developing community-based resources for social, vocational, and leisure skills
    • Maintain communication and links with other Jefferson Center’s programs e.g. Access, Inpatient, Outpatient, Older Adult, and Adolescent programs
    • Ongoing monitoring of assigned caseload, assessment of symptoms to prevent relapse, and follow-up with respite services as needed
    • Coordination for emergency services when clinically indicated/medically necessary; risk/lethality assessment (including inpatient admission)
    • Resource acquisition and assistance with application for benefits and entitlements in collaboration with Benefits/Navigation Team
    • Assist consumers with obtaining resources for basic needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric services
    • Participate in team meetings, clinical staffings, and clinical consultation as scheduled
    • Participate in Evidence Based Practices on the team and in the Center (i.e.: IDDT, DBT, CBT, psycho-education, etc.)
    • Utilize clinical practice guidelines, Center and State Division of Mental Health standards for appropriate service delivery

     

    IF LICENSED:

    • Supervision of staff working on licensure and/or graduate school internship
    • Trains/mentors new hires for Intensive Services Network
    • Provides peer consultation and leadership to support clinical/professional growth of non-licensed staff
    • Provides leadership and assistance with 27-10 mental health hold process, including writing 72 hour hold and coordinating inpatient placement with Access
    • Provides services to third party referrals for clinically appropriate treatment and timely reimbursement to the Center (adheres to credentialing policies)
    • Respond to referrals within twenty-four hours
    • Provide timely follow-up with consumers discharging from the hospital, in accordance with FBHP protocols
    • Timely follow up with new consumers awaiting 1st therapy session. Assess needs and refer to appropriate resources 
    • Engages consumers and appropriate family members and/or significant others in time limited, strength-based treatment to stabilize client and/or avoid crisis
    • Provide back-up coverage completing intakes, assessments, and initial engagement strategies with consumers during Same Day Access times in accordance with clinical practice guidelines and agency policies 
    • Provide back-up coverage for groups as needed
    • Participate in Evidenced Based Practices and other agency initiatives (IDDT, DBT, CBT, Trauma Informed Care, etc)

     

    Other Duties (Productivity Performance Measures, Professional Growth/Development, Relationships/Communication)

     

    • Meet required number of consumer service hours as determined by the Network Director through effective caseload management responsive to the level of ongoing clinical need
    • Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach as well as feedback.  Attends supervision at times and interval agreed upon with supervisor. Attends mandatory in-services; compliance with individualized training plan if required
    • Corporate Compliance including documentation on practice in accordance with regulatory requirements and clinical guidelines 
    • Submits 90% of all Progress-to-Date forms within 3 working days
    • Completes 95% of all paperwork by the due date (CCARS and Service Plans)
    • Ensures that fee collections meet the team goal as determined by Network Director. Monitors client balances and takes appropriate steps in accordance with agency guidelines for collecting payment, rescheduling appointments until a fee is paid and/or a payment plan is in place, adjusting fees as needed
    • Completes 95% of case closings within 90 days after last contact, 210 days for meds only
    • Attends mandatory in-services
    • Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment
    • Maintains effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public. Uses language and behavior to promote dignity and respect
    • Effectively responds to the client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, email and other requests
    • Demonstrates knowledge and skills to develop therapeutic alliance with consumers and to work effectively and with cultural competence with consumers from diverse backgrounds
    • Participates in staff development activities to enhance professional growth 
    • Addresses the whole health needs of the client by ensuring that appropriate releases are in place for physical health care providers, making appropriate referrals as needed when significant physical health needs are a consideration
    • Have an understanding of how trauma impacts the lives of the people being served, so that every interaction is consistent with the recovery process and reduces the possibility of re-traumatization
    • Participate in the Center’s training/educational programs designed to enhance knowledge about Trauma Informed Care, the impact of trauma and trauma recovery
    • Ensure that delivery practices are guided by the 10 principles of trauma informed care
    • Assesses and treats co-occurring substance use and mental health needs within the scope of one’s expertise (e.g. CAC certified)
    • Utilizes a range of appropriate clinical and recovery focused interventions according to clinical need, or refers as indicated, to trauma specific treatment, wellness classes/coaching, co-occurring/substance use services, and employment services

    Qualifications

    • Master’s degree in Counseling, Psychology, or related field required.
    • LPC or LCSW Licensed preferred 
    • Two-year’s experience providing mental health therapy to individuals with Severe Mental Illness
    • Knowledge of Stages of Change model, Motivational Interviewing techniques, and Cognitive Behavioral Therapy skills 
    • Knowledge of Recovery-based treatment and trauma informed practices
    • Knowledge of chemical dependency and abuse 
    • Strong assessment skills
    • Demonstrated experience in crisis intervention
    • Strong communication skills
