    Senior Reach is an award winning, evidence-based program serving older adults aged 60 years and better.  This innovative community based program provides services to older adults who may need emotional support and/or connection to resources. Senior Reach clinicians provide time limited, solution focused treatment in the senior’s home.

    Provide screenings, support groups, and caregiver trainings for caregivers.  Special focus will be the provision of services for caregivers who are caring for individuals with dementia.  The position will be a leadership position requiring high level collaboration with outside agencies and systems.  Position will serve as the content expert for matters related to dementia for Jefferson Center.

    Responsibilities

    Essential Duties:

    • Facilitate screenings, support groups, and trainings for caregivers. Special focus will be provision of services for caregivers of individuals with dementia.
    • Provide education and training for caregivers regarding communication skills, appropriate expectations, change in relationships, and stages of dementia.
    • Assist caregivers with environmental and behavioral interventions to better understand and address disruptive behaviors and create a calming environment.
    • Utilize a team approach with family members/ caregiver, treating primary care physician and/or neurologist.
    • Serve as the content expert for Jefferson Center in matters related to dementia.
    • High level of collaboration with outside agencies and systems.
    • Provide individual in home assessments, individual counseling services based on a strength-based model and develops individualized strength-based treatment plans. Engages consumer and family in the treatment process and goal-setting when appropriate.  Identifies risk factors including lethality and responds to barriers to successful treatment.  Works constructively with consumer to reach agreed upon outcome, and coordinates care with internal and external providers.  Maintains appropriate professional standards and provides appropriate follow-up for clients.
    • Completes Senior Reach & Jefferson Center documentation in an accurate and timely manner and assists with data collection to fulfill any grant requirements.
    • Acts as a representative for Senior Reach and provides outreach presentations in the community.
    • Maintains up-to-date knowledge of community agencies, resources and other services for older adults.
    • Has an understanding of how trauma impacts the lives of people being served so that interactions are consistent with the recovery process.
    • Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach as well as feedback.  Attends supervision at times and intervals agreed upon with supervisor.
    • Submits 90% of all Progress-to-Date forms within 3 working days (team specific).
    • Completes 95% of all paperwork by the due date.
    • Completes 95% of case closings within 90 days after last contact.
    • Maintains effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public.
    • Effectively responds to client needs and problems, initiates and maintains positive interactions, timely response to phone calls, pages, email and other requests.
    • Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.
    • Attends mandatory in-services, compliance with individualized training plan if required.
    • Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
    • Other duties as assigned.

    Qualifications

    Education, Knowledge, Skills & Experience Required:

     

    LSW, Ph.D., or Psy. D. plus two or more years’ related experience required.  LCSW or Licensed Clinical Psychologist preferred. Experience working with older adults in addition to knowledge and skills related to the behavioral health treatment of dementia is desired. Strong knowledge and ability to work effectively with a brief solution-focused treatment model.

     

    Experience must be a combination of clinical practice, or other clinical experiences that provide the tools necessary to successfully fill this position.  Position requires experience with relevant computer applications including electronic medical records, passion and understanding of older adult issues, caseload management skills, good writing skills and presentation/teaching experience and skills.  Ability to maintain a high level of efficiency and compliance with administrative rules and standards.  Strong knowledge base of service community, agencies and providers who work with older adults.

     

    Must have reliable personal transportation and expect extensive day travel in various road conditions.  Flexible work hours may be required occasionally.
