  • Overview



    This position will be co-located onsite three days a week at a geriatric and family practice located in Wheat Ridge.   You will provide behavioral health assessment and treatment to individuals who are patients of the medical practice.  You will also be coordinating services with the practice providers and providing other referrals for the practice's patients as needed. You will utilize an episode of care model to engage individuals seeking treatment and provide strengths based, solution oriented care. You will contribute to the mission and vision of Jefferson Center, a dynamic behavioral health organization that inspires hope, improves lives and strengthens our community and whose vision is a community where mental health matters and care is accessible to all.


    We are looking for someone with a positive attitude, exceptional assessment and therapeutic skills working with adults of all ages, including older adults.  This practice is situated in an area that services those with Medicaid, Medicare, and private insurance. The ability to work within a trusted environment with other providers is key.

    • Your strong practice of  coordination of care across providers in a integrated setting will be key for your success.

    • You will actively individuals in the treatment process and goal setting. 
    • We expect you to work constructively and positively individuals and the practice to reach agreed upon positive outcomes.
    • Client satisfaction will be key, as well as establishing and maintaining strong relationships with the providers will be an assessment of client satisfaction.

    • Current Colorado Licensure as an LCSW, Ph.D., or Psy.D .
    • Your knowledge and experience in a managed behavioral health care model as well as your experience in an integrated health care setting will be beneficial.
    • Your ability to work independently to provide diagnosis, treatment planning, psychotherapeutic intervention, documentation, communication, and coordination with other treating professionals is a must.
    • Clinician already credentialed on 3rd party panels who contract with Jefferson Center is highly desirable.
    • Knowledge of expectations and procedures and ability to work collaboratively and cooperatively with insurance organizations that contract with Jefferson Center.
