  • Overview



    Provide psycho-educational holistic classes and health coaching services to clients with and community members with a variety of presenting mental and physical health conditions within a primary care environment. These services include the creation and implementation of Wellness classes, community outreach and carrying a caseload of health coaching clients.



    • Provide individualized health coaching ground in motivational interviewing and appreciative inquiry to clients across the lifespan aimed at helping them meet their personal health goals
    • Provide active follow up, support and outreach to clients and community members with mental health/wellness needs. This includes telephone outreach, health education and coaching
    • Provide psycho-educational classes aimed at improving whole person health through physical activity, nutrition, and stress management
    • Assist participants to achieve individualized health plan goals through motivational techniques and coaching that are effective, efficient, timely, respectful and person centered for Jefferson Center consumers and community
    • Liaison between physical and mental healthcare providers, and between Jefferson Center and other agencies or providers.
    • Work as a part of the Wellness team to promote client participation in diet, exercise, tobacco cessation and other health change classes and education, and follow up on individual health plan goals and objectives.
    • Participate in team meetings, clinical staffings, and clinical consultation as needed at Jefferson Center.
    • Attend mandatory trainings as required by Jefferson Center.
    • Participate in supervision as agreed upon with supervisor who is the Manager of Wellness Services, come prepared to supervision and take active role in addressing feedback
    • Employee will be held to certain productivity standards that they will be required to meet as determined by management
    • Complete all documentation in a timely and accurate manner and meet all other clinical performance standards of Jefferson Center.
    • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. 
    • Maintain appropriate professional standards and effective interpersonal relations.
    • Effectively respond to patient/client needs and problems, initiate and maintain positive interactions, timely response to phone calls, pages, email and other requests.




    • Minimum of a BA, in psychology, social work, health care or related field.
    • Two years professional experience working in physical or behavioral health environment
    • Passion for holistic treatment and a focus on wellness
    • Knowledge of stages of change model and experience with motivational interviewing
    • Excellent public speaking and communication skills
    • Ability to create relationships and inspire change



    • Master’s degree in psychology, counseling or social work preferred. No licensure required.
    • Registered Yoga Instructor Preferred
    • Tobacco Treatment Specialist preferred
    • Experience in integrated care of mental health field working with the SPMI population preferred
    • Bilingual preferred in English/Spanish
