**BONUS OPPORTUNITY ELIGIBLE**   At Jefferson Center, it is our policy and our mission to be inclusive and mindful of the diversity of everyone who comes through our doors. We are passionate about building a community where mental health matters and equitable care is accessible to all races, ethnicities, abilities, socioeconomic statuses, ages, sexual orientations, gender expressions, religions, cultures, and languages.   The Adult Outpatient program is comprised of several dedicated and passionate clinicians who provide individual and group psychotherapy as well as appropriate case management and/or care coordination to clients ages 18-59. The Transitions team, a subset of the Adult Outpatient Program, provides additional support via bridge sessions and more frequent check-ins, to those clients over age 18 that are high-risk and/or in need of additional support between regular therapeutic contacts.   As part of the Adult Outpatient team, our ideal candidate will work within a team of skilled clinicians to meet the needs of all adults, treating a wide range of presenting issues and concerns. Clinicians are afforded the opportunity to work autonomously to provide sound clinical care, while also having the opportunity to consult with and make connections within the team and beyond. Additionally, as a member of the Transitions team, the ideal candidate will work with ongoing clinicians, crisis center staff, and others to provide wrap-around care for higher-acuity clients. Many of these clients have recently discharged from a hospital, and others require additional support between appointments or between initial assessment and their first session.   While licensure is required for this position, candidates who are within six months of obtaining licensure are encouraged to apply. All candidates interested in sharpening and growing their clinical skills are welcome and encouraged to apply as well. This position is currently a hybrid model, with both work-from-home and in-office time. This is subject to change due to COVID19 circumstances; however, it will likely be a hybrid model long-term.

We are passionate about building a community where mental health matters and equitable care is accessible to all races, ethnicities, abilities, socioeconomic statuses, ages, sexual orientations, gender expressions, religions, cultures, and languages.   This position offers a unique opportunity to serve both adults within the Adult Outpatient program at Jefferson Center, as well as adults and families receiving services from Jefferson County Human Services in the Colorado Works and Employment First programs. The Adult Outpatient program is comprised of several dedicated and passionate clinicians who provide individual and group psychotherapy as well as appropriate case management and/or care coordination to clients ages 18-59. As part of the Adult Outpatient team, our ideal candidate will work within a team of skilled clinicians to meet the needs of our clients, treating a wide range of presenting issues and concerns. Our clinicians work autonomously to provide sound clinical care, while also having the opportunity to consult with and make connections within the team and beyond. The Colorado Works program, also known as Temporary Assistance to Needy Families (TANF), supports low-income families with children in achieving economic self-sufficiency. When people enroll in TANF, they complete an assessment and determine goals within the program and will work with this clinician if interested in mental health therapy. This clinician also supports those enrolled in Employment First, a similarly focused program that supports individuals without children. Because this position serves a vital role at both Jefferson Center and Jefferson County Human Services, this position is co-located at a Jefferson Center office and the Human Services office in Golden. This is a great position for those interested in working with both families and adults!   While a conferred master’s degree is required for this position, both licensed and unlicensed candidates with a range of experience interested in sharpening and growing their clinical skills are welcome and encouraged to apply.

We are passionate about building a community where mental health matters and equitable care is accessible to all races, ethnicities, abilities, socioeconomic statuses, ages, sexual orientations, gender expressions, religions, cultures, and languages.   Jefferson Center is seeking Care Coordinators to join our recovery oriented, phase-based care teams. Our phase-based care model will help people thrive by continuously revolutionizing healthcare delivery, using a data-driven and team-based approach.     Phase-based care is an interdisciplinary treatment approach rooted team collaboration, data, and measurement, where each member contributes their expertise. The latest team-based clinical practices and tools are employed to help those in need get better fast and maintain their health and wellbeing long-term.    We seek to offer our community access to the best behavioral health, substance use, and wellness services starting on the same day they are requested, without exception. We will achieve this while simultaneously creating a work environment where our team has the autonomy, support, and tools to be wildly effective, satisfied, and engaged at work.     As a Care Coordinator, you may be a good fit for our team if your values and goals align with ours: - The people we serve want choice in how they receive services, and it is our job to honor choice.    - The people we serve have complex challenges and a team model best supports solving those complex challenges.    - Showing health improvements via measurable outcomes is an important responsibility of providers, both to the people we serve and to those that fund those services.    Goals  While our use of the phase-based care model has many goals, there are three primary ones worth emphasizing: (1) dramatic reduction in wait times for accessing care and wait times between interventions in the initial phase of care, (2) increase our resilience to industry wide staffing shortages and record high community demand, and (3) decrease staff burnout.     Jefferson Center is hiring positions for the SUD Phase Based team.   Substance Use and Recovery Phase Based Care - Substance Use and Co-occurring Outpatient Phase Based Care provides immediate, non-crisis support and therapeutic services for adults 18 and older and their significant caregivers/families.
